ACH PAYMENT AUTHORIZATION FORM
By filling out this form, you agree to the following: I hereby authorize Caglia Environmental to initiate ACH debits from my designated checking/savings account. This authorization will remain in effect until I provide notice of its termination, which must be received at least 10 days prior to the next billing date. I agree to promptly notify Caglia Environmental of any changes to my account information.
If a scheduled payment date falls on a weekend or holiday, I understand that the payment may be processed on the next business day. I acknowledge that funds may be withdrawn from my account as early as the scheduled transaction date.
In the event of a Non-Sufficient Funds (NSF) rejection, I understand that Caglia Environmental may apply a fee to my customer account. I further acknowledge that all ACH transactions must comply with U.S. law.
By filling out the form below, I certify that I am an authorized user of the specified bank account and agree not to dispute these scheduled transactions with my bank, provided they correspond to the terms outlined in this authorization.
All forms must be signed with a pen or a digital signature with authentication. A digital signature is an electronic, encrypted, stamp of authentication on digital information such as email messages or electronic documents.
A copy of a voided check or an EFT letter from your bank must be uploaded to this form.

5 Ways to Find Your Bank Routing Number
1. Directly on Your Checks: The easiest way to find your bank’s routing number is to look at the first nine digits in the bottom-left corner of your physical checks. See photo example below.
2. Your Bank’s Online Portal: Check your account information through your bank’s online portal or mobile app.
3. Bank Statements: You can usually find your routing number somewhere on your monthly bank statement. Often, it will be somewhere close to your account number, usually in an upper corner of the statement.
4. On Your Bank’s Public Website: Banks will often post their routing numbers on their public websites, so you could also search for your bank’s routing number there.
5. In-Person or Over the Phone: Finally, remember that your bank likely has a wealth of resources to help you in the event you can’t find your routing number. If you’re unsure about which number to use, visit your local branch or give your bank a call to speak with a representative.
